I, hereby described as the Participant or Skater, hereby apply to be a participant of Skateology / Ride Ramps / Control Skateboards Australia's activities (if you are under 18 years of age a Parent or Guardian must sign this form).

Medical Details:

I AGREE THAT MY PARTICIPATION IN SKATEOLOGY AND/OR RIDE RAMPS AND/OR CONTROL SKATEBOARDS AUSTRALIA'S ACTIVITIES ARE SUBJECT TO THE TERMS AND CONDITIONS AND ACKNOWLEDGEMENTS AND WAIVERS SET OUT IN THIS APPLICATION AND BELOW.

TERMS, CONDITIONS, ACKNOWLEDGEMENTS AND WAIVERS

These Terms, Conditions, Acknowledgements and Waivers apply to all activities in which the Participant/Skater takes part in during their lesson/workshop/clinic/event/competition/birthday party with Skateology and/or Ride Ramps and/or Control Skateboards Australia.  It is a condition of the Participant/Skater being accepted into a Skateology/Ride Ramps/Control Skateboards Australia lesson/workshop/clinic/event/competition/birthday party that the following points be read, agreed to and acknowledged by signing in the area provided below.

RISK WARNING AND EXCLUSION OF LEGAL LIABILITY

Skateology/Ride Ramps/Control Skateboards Australia makes safety it's first priority while enjoying the fun and healthy sport of skateboarding, however, there are risks involved in skateboarding as with any other sport.  During a Skateology/Ride Ramps/Control Skateboards Australia lesson/workshop/clinic/event/competition/birthday party, Participants/Skaters must wear full protective gear.  This form must be signed by the Participant/Skater or a Parent/Guardian of the Participant/Skater if the Participant/Skater is under 18 years of age, prior to their commencing activities with Skateology/Ride Ramps/Control Skateboards Australia and prior to their being entitled to participate in lessons / workshops / clinics / events / competitions / birthday partys with Skateology/Ride Ramps/Control Skateboards Australia.

Please read carefully the following acknowledgements and assumptions of risk relating to activities provided by Skateology/Ride Ramps/Control Skateboards Australia:

1. The Participant/Skater warrants that they are fully medically fit to undertake the lesson / workshop / clinic / event / competition / birthday party.
2. The Participant/Skater agrees that they will participate in the  lesson / workshop / clinic / event / competition / birthday party entirely at their own risk.
3. The Participant/Skater acknowledges that the lesson / workshop / clinic / event / competition / birthday party carries some risks and/or dangers including physical injury that may result in permanent disability or death, and economic loss or damage.
4. The Participant agrees to fully comply with all direction given by Skateology / Ride Ramps / Control Skateboards Australia's coaches and officials.

By participating in lessons / workshops / clinics / events / competitions / birthday partys with Skateology / Ride Ramps / Control Skateboards Australia and/or using equipment provided to the Particpant/Skater by Skateology / Ride Ramps / Control Skateboards Australia, either at this time or in the future, the Particpant/Skater agrees:

1. That the Participant/Skater participates in activities provided by Skateology / Ride Ramps / Control Skateboards Australia at their own risk.
2. That the Participant/Skater agrees that in participating in a lesson / workshop / clinic / event / competition / birthday party provided to them by Skateology / Ride Ramps / Control Skateboards Australia that they totally release from all liability whatsoever and indemnify Skateology / Ride Ramps / Control Skateboards Australia and it's employees and agents and contractors against any losses, damages, costs, expenses, liabilities, and judgements suffered or incurred by the Participant/Skater or any other person in respect of said activities.
3. That Skateology / Ride Ramps / Control Skateboards Australia has no liability to the Participant/Skater for personal injury, disease or death arising from the services and/or equipment that Skateology/Ride Ramps/Control Skateboards Australia provides to the Participant/Skater.
4. That the Participant/Skater will comply promptly with all directions given to them by coaches and other officials of Skateology / Ride Ramps / Control Skateboards Australia, its employees, agents and contractors during any lesson / workshop / clinic / event / competition / birthday party.
5. Skateology / Ride Ramps / Control Skateboards Australia may take photos/videos during Skateology / Ride Ramps / Control Skateboards Australia's lessons / workshops / clinics / events / competitions / birthday party and the Participant/Skater or Parent/Guardian of the Participant/Skater (if under 18 years of age), agrees that photos/videos taken at such times may be used for promotional or marketing purposes by Skateology / Ride Ramps / Control Skateboards Australia.
